The Huria community care team provide a Health Social Service to all families and
individuals who reside in Tauranga.

About our Services
Our aim is to help improve the quality of life for whanau and individuals who may need
assistance or may be at risk.

We can provide advocacy and consultation on:

Health and Welfare issues
Regular monitroing of needs
Access to support services and monitoring of those services
Support coordination with Community Health Providers
Support while in Hospital and upon discharge

We discuss and complete with you a 'Health Care Plan' that is best suited to your needs.

Our Health Promotion Activities Include:

Asthma	Breast and Cervical/Cancer Screening
Diabetes	Menopause
Life Changes	Nutrition Fitness and Mobility

Traditional Maori Healing

We help individual, whanau members, kaumatua, koroua and kuia, pakeke, matua, rangatahi, tamariki, mokopuna who are in need of our services.

We offer a hands on Visiting, Monitoring and Assessment Service by trained and culturally qualified personnel.
